A document to be completed and filed by the debtor on a designated official form listing information about the debtor such as: the debtor’s gross income for the two years before the bankruptcy filing; transfers by the debtor to creditors in the 90 days before the bankruptcy filing and transfers to creditors who are insiders of the debtor within one year before the bankruptcy filing; identification of all bookkeepers and accountants for the debtor for the six year period before the bankruptcy filing; a statement of the dollar amount of the debtor’s last two inventories taken before the bankruptcy filing; and a listing of the debtor’s partners, officers, directors and significant stockholders.
